OPT - Post-Completion Optional Practical Training Request Form

UIS has transitioned to a new portal for all request forms and inquiries. You will submit your request or inquiry via the student portal. You should have received an email with your username, instructions, and a link to the portal log-in page. Please find this email and follow the instructions.

Click on FORGOT PASSWORD and initiate a password reset. A pin will be sent to the personal non-student email you provided earlier. If you have a new personal email and need the pin to be sent to your new email, please submit this form and UIS will have the pin sent to the new email.
