STEM OPT I-20 Request Form

To apply for STEM you need an I-20 with a DSO recommendation. To request this I-20, please submit the following items in a single email to the UIS with the subject line STEM OPT REQUEST:

• This STEM OPT I-20 Request Form
• Scan of the completed I-765 - Please note that question 1 Reason for Applying should be "Initial Permission" (Option 1a) because this is a different category.
• Scan of the completed I-983
• Scan of your Job offer letter (issued within 4 months)

Upon submission of all required documentation, the UIS will then generate your STEM OPT I-20, which you will send by mail to the USCIS along with your STEM OPT application packet. Please avoid changing employers while STEM OPT is processing as it can add to further delays in processing. Federal regulations require F-1 students on STEM OPT to report to the UIS every six months regarding your STEM OPT job and location.

I authorize the UIS to create a STEM OPT Request I-20 for me, which I am responsible for shipping to the USCIS Service Center. I understand that USCIS considers the legal burden for the correctly-filed STEM OPT application to be on the student. The UIS will provide the STEM OPT I-20 with DSO recommendation on it, but it is solely my responsibility to properly prepare the correct application packet and file the application with USCIS in a timely manner. I understand that adjudication of STEM OPT work authorization lies with USCIS, not the UIS, and that the UIS will not take responsibility if the USCIS denies an application due to errors made by me.
