Use this form to request an official letter from the University to certify that degree requirements have been fulfilled. To receive a letter of degree completion, all fees owed to the University must be paid in full. Please note: A letter will only be issued after all degree requirements have been completed and reviewed by the Registrar’s Office. All letters will be sent via email. If you need a paper copy please enter the mailing address in the Additional Notes field.
