On June 16, 2021, the University announced that COVID-19 vaccination would be required for all students, faculty, and staff that will access the University's campuses and who did not obtain an exemption. On January 4, 2022, the University announced that the COVID-19 booster dose would be required for all students, faculty, and staff who who are eligible to receive a booster dose and will access the University's campuses, but did not obtain an exemption.

Fully Remote Students
Students who are fully remote - meaning, they do not intend to come to a University of New Haven campus at all during the semester - do not need to meet the vaccination requirement until such time as they decide they would like to return to campus. If you decide later that you want to come to campus, you will need to be fully vaccinated (including booster dose) or receive a vaccination exemption before you are permitted on campus.
You should NOT choose this option unless all of your classes are designated as online and you will definitely not be accessing the campus(es).
Please note that the only exemptions available to employees, including student employees, are medical contraindication or religious exemptions. If you plan to work for the University as a student employee, you cannot do so under a remote exemption - you must be vaccinated, or receive a medical contraindication or religious exemption.

Fully Remote Employees
Employees who are fully remote and who are currently fulfilling the expectations of their role off-campus, are not required to meet the COVID-19 on-campus policies and requirements at this time. A fully remote employee was previously authorized by their supervisor to do so and never has occasion to visit the campus.
If the needs of the University change in the future where 100% remote work is no longer feasible, employees will then be required to comply with all of the existing COVID-19 policies at that time.
Please recognize that employees should NOT choose this option unless supervisory approval has already been received and that you are authorized to perform all of your duties remotely.

STUDENTS, PLEASE NOTE: If you have previously been approved for a vaccination exemption, you SHOULD NOT request that same exemption again in subsequent semesters.
Only submit this form if you need to change an existing exemption, or have never been approved for an exemption.
For example, if you were studying remotely during the fall semester but need to come to campus in the spring and are not vaccinated, you would need to change from a remote exemption to another type of exemption that allows you to come to campus.
