This form serves as the approval mechanism for employees who are requesting university-sponsored travel.
Personnel in departments that conduct frequent travel such as Admissions and Advancement should speak with their supervisor and follow their normal travel procedures instead.
• Before proceeding, a Travel Authorization Form should be completed in its entirety, signed by the traveler and also contain the signature(s) of all required approvers. (See the University Travel Policy for more information.)
• Submissions will be reviewed by the designated VP/Provost and the Business Office.

Background Check Policy
You must read this policy and adhere to it, in correspondence with the Human Resources Department.

What is a CSA?
Since you are traveling with students you are considered a Campus Security Authority.
Campus security authority is a Clery specific term. It identifies University employees responsible for student and campus safety. And, it covers every person with significant student and campus safety responsibilities. Every official whom students would want to approach with crime information can be a campus security authority. The person should also have the authority to make decisions.
