PLEASE NOTE: This form should only be submitted if your chosen first name is different than your legal first name.

The University of New Haven recognizes that many of our community members use first names other than their legal names to identify themselves. Therefore, any student, staff, or faculty member may declare a chosen first name to be used wherever possible across campus.

We strongly encourage all community members to familiarize themselves with the University's Gender Identity/Expression Policy. This policy includes further details regarding the University's procedures for chosen first name requests, including updating the University Identification Card. This policy is accessible through the Myatt Center for Diversity & Inclusion page on myCharger (login required).

2.) Your chosen name will be added, to the extent possible, to all lists and rosters that are generated and distributed internally. Note there are numerous databases used at the University of New Haven and there is no guarantee that all communications will reflect your chosen name, and in some cases your name of record will also be included on them.

3.) You are permitted to obtain a University Identification Card (Campus Card) with your chosen name. To process this request, please contact the Dean of Students Office.

Important considerations:
1.) When you submit this form, the inclusion of your chosen name, as explained in Section A, will remain in effect until and unless you rescind this form in writing and your request has been acknowledged by the Registrar's Office.

2.) As your legal name is a permanent element of your University record and held by many offices, there may be occasions when faculty, staff, or other individuals use your legal name instead of your chosen name. There are also purposes for which only legal names are used, for example student accounts in financial aid and bursar's office. Other examples may be health services for insurance purposes, campus police reports, and ad hoc rosters. Every effort will be made to minimize such exceptions.

3.) Per the University's Gender Identity/Expression Policy, there are certain University offices, and circumstances, which require the use of an individual's legal name. The following offices, and circumstances are listed below:
