Students using VA Educational Benefits should submit this form once their schedule for the upcoming term is completed. Submission of this form indicates that the student does not plan to make any further changes to their schedule and is ready to have their certification submitted to the VA.

You will be responsible for submitting a Notice of Change form should you make any changes to your schedule after the submission of this form. Please be aware, any schedule changes made after the initial certification may delay payments or monthly housing allowance.
